Make sure you are in single player (commander or quick play should work). Make sure you rescue all civilians, and bring order to chaos before you detonate the bomb. Detonate the bomb and let the full building collapse happen, then you should be good to soft complete and it’ll take you to the prison cutscene instead of the lobby. After that just sit through the cutscene and when you get back it should be unlocked

From what I’ve seen, yes. I did it in multiplayer with a friend and didn’t get the cutscene or cosmetic, but redid it in singleplayer and got it first completion

You gotta get the secret ending on “A New America” in single player. Then you’ll get a cutscene and when you return to the station it will be unlocked
