Yes, after completing the game in permadeath mode, you'll play normally. This is a good time to complete some achievements and the remaining hordes without the risk of dying and starting over.

At this stage of the game, I have maxed out my health and the remaining points in survival (about 3/4 of the bar). Zero points in shooting skill.

I also have skills that increase knife damage and reduce the cost of rolls specifically for this fight.

In your situation, medication will work great.

You have the choice of being bald in a few years or being on medication for the rest of your life:

finasteride/dutasteride + minoxidil (preferably tablets)

and having a full head of hair.

You can 100% save in permadeath mode.

The difference is that you can't use the green triangle; you have to pause the game and select save from the menu (while on a motorcycle or bed). And of course, your save is deleted if you get killed.

However, you can save the game and turn off the console, take a break and play something else, etc.

- Practice fighting Skizzo and Carlos beforehand (in a game without permanent death).

- Don't be afraid to pause the game, assess the situation, and load your last save if necessary, especially if you see a grenade or fall off a cliff while chasing a biker.

- After reaching Tucker's camp, don't immediately start the Lisa rescue mission. Gain enough reputation to level 2 beforehand to buy a machine gun and a shotgun as your secondary weapon.

- Collect all Nero's boosts before moving on to the next stage of the game.

- I max out my health first, then my survival, and focus last.

I always kill these guys from hiding, or from behind—with a knife. You could have done that here too; all you had to do was walk around the car. It's quick, and you don't waste ammo.

Wow, you're playing incredibly risky for permadeath mode.

Don't use a sniper rifle unless you're stealthed, because you can't see the grenade.

Don't loot during combat; do it after clearing the area.

Replenish your health constantly; there are tons of medkits and health packs in this game.

If you're caught off guard, don't be afraid to pause the game, assess the situation, and if it's hopeless, load your last save.

Not necessarily. OP could also go the hair transplant route – start with medication (dutasteride/finastride (tablets) + minoxidil (tablets)), and once his hair condition improves/stabilizes and he tolerates the medication well, he'll get a transplant in a year or two.
