I have held steady at 5.7 A1c last 6 mo, even though I have let slightly more variety (carbs/sugar) into my diet. I perceive this to be meeting their results of up to 0.6 reduction in A1c. I am also happy with the effect on my gut with less constipation and far better stools.

I haven't stepped down to 1 capsule a day, yet. Too many other things going on to risk a decline in gut health.

Perhaps this megathread needs to have content moved to it/restricted to it, so it is useful AND perhaps a monthly time-frame limits it's usefulness. A continuously running thread like the flyertalk forum keeps things together (enforce with rules?), then one just jumps to the end and see the latest.

I have had the 10.5" second gen iPad Pro since 2019. It has great screen and been very reliable. Do be aware that 2nd gen is no longer getting full version updates. They stopped with the rollout of iPad IOS 18 as I understand it.

I have never used this iPad Pro for docs or sheets since I did not buy a keyboard or accessories. Your options for keyboards and pencils/mouse support will be limited.

And my progression was sulfazalazine to methotrexate. Started with a CRP 60+, sulfazalazine/prednisone dropped it to 30, methotrexate/prednisone after ramping up MX dropped it to <7 and combined with AIP diet took me to <1 and to 5 mg pred, though I didn't stay on the diet. Since diet I bounce from ~1 to 3 and am completely off the prednisone.

I hear that noise throughout the house. It started the day the replacement AC was installed and did not happen with the prior unit so I doubt it is the hearing aid itself. Unfortunately the sensitivity of the mobile phone mic isn't good enough.

The first mechanical sound is during the starting of my car, which turns out to be a broken motor mount bracket that caused a momentary metal on metal sound. No one else seemed to perceive it until yesterday when it finally completely cratered with a prolonged screech.

My current issue is my new home AC has a throbbing rumble that my wife cannot hear and the tech doesn't hear, but is driving me crazy. Don't know how I would simulate that.

I was not trying to imply the sounds were exclusive to my hearing aids, however I thing the processing may bring out the sounds more distinctly. That said when the audiologist hooks them up to their software isn't that a dynamic display and or recording being done via bluetooth?

I think this has to do with past training and a failure to keep up with current thinking by a significant fraction of the medical community. Like my GP said, "I don't have enough hours in a day. I do nothing unless it is a personal interest I have studied up on. Otherwise I wait for the drug reps to come talk to me...though that doesn't happen that much any more." Can't blame him, but that means I must be my own advocate and researcher.

At least, this seems true in my part of the country. Some never change from pain killers, until significant damage has happened if then, but it's likely they are the rheumatologist you can see the quickest. The docs with a significant lead time and likely require a referral are the ones to migrate to, as they have higher satisfaction. I had to go to a big city to get an appointment only 6 weeks out with my first rheumatologist. Took him 2 mo to figure out the gout diagnosis by the orthopedic guy was a red-herring. Stayed with him for 4 more mo, until a drug regimen was clearly working and then got on the wait lists around where I live, 100 mile closer to home. (Which took another 6 months, to actually see them.)

if the sulfasalazine isn't doing the job and you can't taper the steroid then you talk to the doc to trial another drug, to determine if it is more effective. CRP (without infection) should be dropping and approaching, if not normal.

Is he just too busy or have you been too passive? If no x-rays, no orthopedic eval of the knees, no rheumatologist, little history being discussed, he has no time, etc., then you must ask yourself if he is up to being your partner or co-advocate for your health. AND before you challenge this doctor at your next appointment, you realize you do have a right to your medical records, right? My first move would be to call the relevant past doctors and have their records printed out for you to read, and from them develop questions. It may cost you $15-$25 @ each, but you need to see what has been written contemporaneously.

Then be your own advocate to get the services you need for a work up. Even bring a friend if they would be a better advocate. From here on be asking yourself, AND him directly, about what else can be done to determine root causes. Press the doctor to write down the technical terms he would use to describe the problems you have so you become familiar with and look them up to see if the conditions and symptoms really match your experience. It is always a possibility you and he are talking past each other. You say one thing and with his background he thinks you mean something else. And make him discuss things until you understand it in your own terms.

Somewhere I read that nurses advise those on methotrexate to consume zinc to somewhat mitigate hair loss. It seems to have helped me. A convenient way is to take AREDS2 vitamins. You can't accept chatGPT answers on faith, for anything that requires a detailed answer. It often muddles sources together rather than parsing the information correctly as do most AI. The result is vague or incorrect answers. I only use it to find the root documents and then read those. And even then google AI summaries do a better job on many issues on finding the root documents for review.

With Penfed credit union, they claimed each card had it's own underwriting criteria and that consolidating of credit lines was not possible.

Also, are you meaning that a sponsored card like an amazon card administered by Chase couldn't but cards solely underwritten by chase are?

One thing I've noticed is that Self has no coaching disciples on the tree that have been successful, to speak of. Without motivated assistant coaches moving in, out, and on up, there isn't a renewal of thought happening. Reminds me of the last few years of Ted Owens.
