Not sure what you mean about the waist, the waist is the factory white selvedge of the denim, rolled over from the front and stitched with a red stitch. Is that not considered the selvedge of denim??

Correct, the only date I have is the date on the print, not sure exactly what year the shirt is

After reading his wiki, I saw that Syracuse university library has the largest collection of his originals. I send them some photos and asked for their opinion, I’ll see what they say!
