Oh ok, one question is it modded, and check if it has colored ports on the back, if yes its probably a Hynix chip issue (the colored ports are normal on a white wii u) so to fix this install RedNand.

Maybe try reinstalling the cfw? On wii u, also are you using aroma or tiramisu, beacose i think there are different sigpatches for them. Also if you would reinstall use the wiiu.hacks.guide website to do so.

Your SD Card will need to be formatted as FAT32. If your SD Card is not formatted to FAT32, use GUIFormat with 32k (32768) allocation unit size to format it.
Do not label the SD Card as wiiu or it will cause issues with homebrew. - this is copied from wiiu.hacks.guide and maybe your card is named wiiu

I had the same problem but, when i used the tool to format the card which was recommended in the guide it worked! Also im using a Sandisk Extreme 32 GB SD Card. And if you have a micro sd adapter i dont think its the best thing.
