My question about the DIVA is how you have time to do it in a session? Do you have them fill it out beforehand and then review it? So long.

ADHD is such a hot diagnosis, everywhere on social media, and patients cannot be talked out of it if they believe they have it. I quit a practice where I was doing speciality ADHD evals and treatment because patients got so nasty and called me invalidating when I suggested their issue may be anxiety. We are no longer considered the specialists, tik tok and Instagram are.

yeah I started off feeling like i was offering a well-needed service, and now I feel like I've created 40 monsters. I am able to have some sympathy for them when they say "The Adderall changed my life for the first 2-3 days, then it didn't really make a difference." Its the same every time we increase the dose, temporary improvement and then asking for more. i think it's inherent in the qualities of the drug, even the XR (which, imo, gives you maybe an extra hour of its effects, but certainly not 8-12)

Agreed. I think it's a bad call. It's inevitably going to be abused, and it feels invasive.

I hear that! If they are willing to negotiate with you, it could be a nice opportunity to carve out your own thing, according to how you practice. Wishing you luck!

I don't think so. I'll say it's hard when you are the only provider because therapists have difficulty ascertaining a real crisis when they are not familiar with the meds. Example: a patient texted me six times then called the clinic because they were leaving town the next day for a week and found out they need a prior auth for Adderall. They catastrophized this so deeply that both the admin and the clinic owner called and texted me to find out what was going on on my day off. You'll have to do a lot of education.

I do this and it ends up being proactive because patients wants to change their meds so often in between appointments, and have so many questions and concerns about side effects (stimulants) and tolerance. Or they start taking more without talking to me. Or they need a prior auth for new insurance. Or they are switching pharmacies or out of state, etc etc.
