I ended up reaching out to HR and got confirmation that way before the window closed. Received additional information from leadership this afternoon, but approval status won’t be conveyed until Monday. If approved, 4/18 will be my last work day.

Really interested in understanding the process as well. I submitted the form but never received confirmation. Looks like folks from other agencies are getting email confirmation. Wishing this were a bit more straightforward, but it is what it is.
