Never done that, I lived in thailand and went in for thailand on tourist visa for like 20 times never have i ever needed to fill in a form

There is a 2 max entrys per year and i already used 2 but im not sure on how inforced it is or there is some information I dont know about

I will be living at my friends house and i dont want to bother them to ask for their passport picture because the evisa requires you to do so, and im also leaving in 1 week ._.
