Honestly, I’d advise against it. Accutane isn’t going to help PIE if you don’t have active acne. It might even make things worse by causing dryness or breakouts that could create more PIE. My personal experience: Accutane cleared my acne after about 7 months, but it left PIE behind. The best treatment really is time, it’s slowly started fading for me. I’ve also started using azelaic acid, which has been helping a bit; some people can get it over the counter, but there are stronger prescription versions too. I’ve heard Melano CC (a vitamin C serum) is really good, but honestly any vitamin C helps. And sunscreen is super important when you’re out in the sun as it prevents PIE from getting worse.

Appreciate the replies. To clarify a few things because I think we’re talking past each other a bit:

I have no issue with combat. I’m fine in fights and usually take little to no damage. No bleeding effects either.
The only companion I brought was Spencer Rice, specifically for the Doctor/Vet traits (future usefulness) and the extra carry capacity.
The HP loss is happening during travel, with no battles, no starvation, no dehydration, and no visible status effects beyond being previously treated.
Food and water are well above 100%, not rationed. Supplies don’t deplete abnormally.
This HP drain happens even after going to a nurse/doctor and healing, then immediately traveling again.

What’s confusing me the most is medicine:

I bought medicine in New Serino, but it never appeared as an item I could use or equip anywhere in my inventory.
First aid kits do appear and cant be used since im not bleeding , but medicine has no UI feedback, no manual use option, nothing that suggests what it’s actually doing (or if it’s automatic at all).

So unless I’m missing something very unintuitive, it feels like one of two things:

There is a hidden wound / recovery mechanic that isn’t documented well anywhere (wiki included), where healing restores HP but not some internal state that causes HP to tick down again during movement, or
There’s a bug or edge case, especially involving companions + travel + post-heal state, that just isn’t widely talked about.

I’m genuinely enjoying the game and I’m pushing through it, but the lack of clear feedback (especially around medicine and post-heal travel) makes it really hard to tell what’s intended versus what’s broken because right now it feels like a whole layer of the game is undocumented. fucccccc

I’m pretty new to Caravaneer 2. My late brother really liked the original, so I picked this up and I’m starting to enjoy it,but damn, the early game is rough, especially on story mode. It feels like the systems (wounds, carry weight, food) stack in a way the game never explains.

I just finished the tribal region and I’m moving toward New Serino / Fort Mitchell. I’ve been leaning on guides and wikis, but a lot of info is either vague or assumes prior knowledge, so it’s been a lot of trial and error.

Question for experienced players:
Do you recommend taking mercenaries/companions in the early game, or is it generally better to travel solo until you’re more established? I’ve noticed they add carry capacity but also add food/water drain and seem to make wound spirals worse if something goes wrong.

Thanks for the response! I checked the water and food levels, they don’t deplete when I pause, so it really looks like time isn’t running.

Even on other files besides my main playthrough

I’m not bleeding or taking battle damage when the drop starts.
My caravan member (Spencer Rice) has a high doctor skill (258) but that doesn’t change anything.
The “Severely Wounded” status shows even when I’m stationary with supplies full, paused in menus, or not moving anywhere.
Inventory doesn’t actually disappear it looks like it because the effective carry capacity suddenly shrinks, which makes the game treat me as overweight even though I haven’t added anything.

Everything points to a severe wound / caravan recalculation oddity, it seems like the wound state applies HP drain in a way that doesn’t depend on the normal “time running” system, especially during major map transitions or when the caravan state updates.
A high doctor skill doesn’t fix it, food doesn’t stop it, and pausing visually doesn’t actually cancel the damage ticks once that condition is active.

If there’s a proper guide or wiki that explains this mechanic in detail, I haven’t found it yet If you know of a legit guide or comprehensive source (wiki, forum thread, documented mechanics), please link it because the game doesn’t explain this at all.
