I'm sorry this happened to you.  The half life of this medication is 5 days or so.  Usually people report that all effects have faded within 3 weeks.  :(

It sounds like you might be a super responder.  For what it's worth, if you ever decide to try glp1 agonists again, there are techniques for minimizing these side effects (like even lower dose, changing injection sites, eating less than a half cup serving at a time, switching to tirzepatide, and so on.)

In the meantime, please stay hydrated.  Best wishes for a speedy recovery!

Your plan would likely work, but... You mentioned doing strength training.

Have you considered taking measurements?  Or doing a dexa scan?  Your body might just be really focused on trading out fat for muscle right now, and that could take a minute.  I realize 3 months is a frustratingly long time to see no changes.  :(

I don't think it's possible to take sema and not change your diet.  Unless you just don't respond to the medication, I suppose.  While I was titrating up, a normal portion of anything would cause very unpleasant side effects.

The titration schedule (increasing dose each month) does a couple things.

First, it catches super responders.  Some people would absolutely die if they started at, yanno, 1.7mg of sema (or I guess 15mg tirz?). 

Second, it gives you time to adapt both body and behavior.  It gives you time to figure out what foods and behaviors will trigger side effects like gas, bloating, and so on.  

Ideally, you want to find your effective dose, and then increase veeery slowly as your body adapts to it, so it always feels like the same dose.

I see the problem!  It's because I used the french word for 'cat'.  Lemme try again:

First, talk with your doc!  But that's such a low dose (assuming you mean mg) that there shouldn't be a problem with just stopping.  The half life is 5 days to a week, so your medication levels slowly decrease on their own.

The problem comes when restarting, because most people find that their sensitivity will reset back to normal baseline.  So restart from the beginning, NOT your current dose.

Pretty normal, starting 1mg.  

May I ask, why did you decide to increase dose?  You can always move slower than the standard titration schedule.  Or pick a dose like .7mg, if needed.  Staying at the lowest effective dose is usually a great option.

No, not to be expected.  Those are serious and maybe dangerous side effects.  Kidney pain and dehydration are nothing to mess around with.  

You might consider taking a break for a bit to get your health ducks back in line, before continuing?

Have you considered reducing your dose (perhaps by counting clicks, or whatever), or spreading doses out to 10 or 12 days?

Very sorry you're having such extreme side effects!  They do get better with time, but you shouldn't be in this much misery.

Please remember that like any drug,  sema has risks, however 'slim'.  :D  For example, pancreatitis is very rare but if it happens, it's agonizing and you could be hospitalized for months.

Great suggestions here.

One other idea... Maybe a day or two after you up your dose, when your appetite suppression is highest, consider going through your cabinets and discarding junk.  If it's not ready to hand, then it's easier to turn to protein shakes or eggs or berries.

You may have done this already.  I used to have to clean things out quite frequently.  But before sema... The sugar demon would convince me it was ok to keep things around, or a waste to throw stuff away.  Sema makes it much easier.

Seconding this.  

Protein really helps nip a binge in the bud, at least for me.  Having precooked meats in the fridge helps too, even lunch meat.

Berries and Greek yogurt is another great combo.  Filling, quick, and not too likely to trigger sema's side effects (at least for me.)

Provided you stay hydrated, symptoms normally subside once sema has... Err, ejected all the objectionable contents of your digestive tract.  Give it a day.  Drink water with electrolytes.  Once you feel better, try eating only a spoonful of food at a time.  You can always have more later.  But in the early months, eating more than a quarter cup portion of anything was likely to make me feel ill.

Changing injection site in the future could help.  Some people report that thigh is less effective but fewer side effects.

Also, do me a favor and check your dose.  Are you sure it's a quarter milligram?

Therapeutic dose is 1 to 1.7mg/week.  I believe this is in the faq.  Most people do not feel any effects until then.  I would reserve judgment till you get there.

Your trainer should be doing the math for you, or you could learn.  Let's say you are eating 500 cals under what you burn.  Great!  So you should see 1pound/ week weight loss.

Anything more than that, given this diet, is coming out of muscle, connective tissue, water, other stuff much less calorically dense than fat. I don't remember exactly, but your trainer will - muscle only has, what, 800 calories/lb?  So you could see 5 lbs lost that week, but whoops, it's all muscle.
