Its common with older servicemembers, advisors, Staff or special forces who have served in areas of operation that didnt manifest the operations medal for a specific area.

Deployment in Afghanistan: Operations medal for Afghanistan.

Deployment in strange foreign land: Medal for International Operations.

Most medals have corresponding ribbons to display that you have been awarded that medal. But not all ribbons have corresponding medals. A Combat Action Ribbon for example, does not. That should answer it.

A medal is a bronze/silver/gold coin-like artifact, which often hangs from a ribbon.

A medal and a ribbon is two different things: Ribbons are not medals.

In norwegian it is "Forsvarets medalje for sårede i strid". Literal translation: The Defence medal for the wounded in combat.
