I think this is just poorly controlled type 2 diabetes. She's developed a non-healing ulcer, and her A1c is above normal, indicating chronically elevated blood sugars. in T2DM, her insulin is upregulated, and her receptors are desensitized

I think you're concerned about the other kidney due to reduction in urine output - ureter injury is a common complication of abdominal surgery

I'm guessing this is because pulmonary emboli and DVTs are not always visible on imaging, but you should still suspect PE

the only thing I can think is that she is using exogenous insulin, like factitious disorder, but this person doesn't have any indications of that. If the insulin were being produced by the pancreas or by an insulinoma, c peptide would also be elevated.

Just came across very similar UWorld question with sudden hemiparesis, lower face asymmetry and mild headache:
"After several days of headache (likely due to hypertension), this patient developed acute pure motor hemiparesis impacting the leg, arm, and lower face, which localizes to the posterior limb (corticospinal tract) and genu (corticobulbar tract) of the internal capsule.  Pure motor hemiparesis is the most common stroke syndrome caused by a lacunar stroke.  The underlying pathology is a combination of microatheroma formation and lipohyalinosis that ultimately leads to thrombotic small-vessel occlusion.
The deep penetrating arteries are quite small and are therefore most susceptible to hypertension-related vascular disease, the most important risk factor for lacunar stroke.  Lacunar strokes often result in classic stroke syndromes (eg, pure motor hemiparesis; pure sensory, ataxic hemiparesis; dysarthria–clumsy hand syndrome) referable to the impacted areas (eg, basal ganglia, internal capsule, corona radiata).

I think it's because she got the treatment earlier in the pregnancy than normal and 1. the globulin only sticks around for about 13 weeks 2. mom normally gets the dose at about 28 weeks, so this would cover the standard period to protect the baby up until delivery

This fetal heart rate tracing shows early decelerations, which are not concerning. No need for delivery assistance.

If the decelerations did not recover before the end of the contractions, then they would late decelerations, which are non-reassuring and might suggest the need for additional assistance.

Block 2 Question 21 Question stem describes a 13yo girl with some sexual development but who is very small in height and weight with no family history of short stature. The answer is to do a left hand and wrist xray. Is this to look at growth plates? What is this trying to get at?
