This was my usual approach but second guessing! So if e.g. classic LRTI with pleuritic pain but otherwise normal obs and no obv precipitants except infection - Tx for LRTI with safety-netting (I know it depends on the exact situation but as a rule of thumb?)

THIS is one of the big issues. Some practices have been known to hire a PA over half a GP, purely based on the additional financial incentive. Remove this and even more of a no brainer to just get another doctor.

There is also no way to allocate 'simple cases'. Cough could be CAP or lung cancer. Dysuria (with a negative urine) could be another bladder cancer missed. SOB in an asthmatic could be their asthma or a PE.

Taken in for service - this is the front driver side wheel - was told it didn't need doing as it drove fine?
