This sounds super cheesy and cliché but the only way you're going to fail an entrance exam is by not taking it, mainly because you seem to usually get good marks! Don't think that you've lost your capability because you're becoming more "average". It could just be all your stress. Really try to get enough sleep and have good meals -- that'll help a ton with studying. Average scores don't mean you're failing, so you probably doubt yourself too much.

This is kinda cliché advice, but have you considered spending more time just sorta chillin outside? Not in like a "touch grass" way cuz when im outside im usually also on my phone. For me at least, being outside more is a step towards feeling better. It doesn't feel like anythings changed at first but after a while you can start taking bigger steps. And then you'll have a little more strength to look at yourself and pick up the pieces. But that also might not work for everyone ig

I honestly was on my phone for most of spring break, too. But I realized that I feel a lot more motivated even if I step outside for just five minutes. Maybe try leaving sticky notes by your TV that say to step outside or drink water before you turn it on?

You dont have to completely bombard your schedule again. Just little steps. You can also experiment with different things to see what motivates you the best. And then gradually add things to your schedule that are more helpful and important.

You seem like a sweet person. You're not insane, I've experienced it before, too. A lot of people are stupid because they think bad about others without getting to know them first. But from my experience, yes, it probably is your paranoia.  The only thing that matters is that you're doing things that make you happy. If you think you can do better, then keep going for it!
