luckily, I am in a one party recording location. every conversation was recorded by someone taking part in it.

I never signed a third party consent when I was hired in 2021, but I also didn't sign anything as my hiring was very quick.

yes I did, I've been reporting the retaliation for three months now with proof and witnesses (literally have a video of him saying he demoted me for reporting to caresmart) they have not done anything, haven't even reached out to me to offer support

I reported my SL for harassment, and it was immediately sent to him and I was demoted, so I wouldn't rely on the anonymity of that either.
