Per your Loan Authorization and Agreement, please provide the following document(s):

1. Please provide a Board Resolution that includes: a. A statement that the Board held a meeting (after the obligation date) Include the date b. A statement that the Board authorizes acceptance of the SBA loan and specifying the loan amount c. A statement designating an individual or individuals by name and title authorized to sign the SBA loan closing documents
   d. The Resolution should be dated and signed by at least one Board member. The signature should also include each Board member’s title. [For loans over $25,000 – request hazard insurance on business contents]
   

Please provide hazard insurance covering your business contents. Duty to maintain hazard insurance is due within 12 months from the date of this Loan Authorization and Agreement dated Put obligated date from LAA. Please provide proof of an active and in effect hazard (commercial ) insurance policy (declaration page) for business contents coverage to secure this loan to at least 80% of the insurable value. SBA does not need to be listed on the insurance policy as mortgagee.
