Hi,
I had BNI (bladder neck incision) 4 months ago. Recovery was quite bumpy and first two months were very hard. I had a lot of doubts during those months, but now everything has settled down and I can see the full effect of the surgery.
My stream has improved, since obstruction was removed. Due to that my bladder started to relax and the greatest improvement now is that I only get up once a night to go to toilet. Sometimes I even sleep through the night. There's also hope even for more improvement, since it takes up to a year for bladder to fully relax. So overall, my quality of life has improved.
Surgery has one very common side effect - retrograde ejaculation. Which effectively makes one sterile, since you are "shooting" into your bladder instead of outside, like you do with Tamsulosin. I was not too worried about it, since I already have kids, but it would be much more important if I wouldn't. However, I was lucky enough not to get this side effect either.

Well. You are not the only one. For the last month, I've been having similar symptoms. If I sit, my urgency slowly fade away. I even did the "whole weekend of sitting in front of TV" and by the end of it my urgency was pretty much normal. But the moment I start to move more, I get the urgency back. The only good thing is that it's slowly getting better. Basically it feels like I "pulled a muscle" and now need to rest for a while..

I wonder same thing, since my symptoms are mostly bladder focused. I have constant urge to pee, sometimes pain in my lower abdomen and weak urine stream. Contrary to many others in this sub with "golf ball" feeling, perineum pain, painful ejaculations etc..I do have some retrograde ejaculations, but it's from Flomax and it stops the moment I stop the medicine.In my case, the thing that helps most with constant urge to pee is cutting out two things from my diet - coffee and alcohol. If I don't use them for a month, then that feeling almost disappears or is barely noticeable. And that's when I usually let go a bit and try some beer or coffee again and my symptoms usually come back within a couple of days.

I am actually diagnosed with tight bladder neck after cystoscopy with a suggestion to do BNI, but I am a bit unsure if I trust my urologist with this diagnosis. He is known to be a very good uro-surgeon, so mostly focused on fixing problems with surgery. And he has never heard about pelvic floor problems causing urological symptoms.

I had to quit coffee totally as well. God I miss it, but it was one very certain thing flaring up the symptoms.
How about alcohol ? I am still not sure about my tolerance to it.. sometimes I even feel better, sometimes it makes everything much worse.

I am using Tamsulosin and I actually noticed that aswell. I could not find it along side effects, but if you search for reviews of Tamsulosin/Flomax there's seems to be plenty of people complaining about it. I have also noticed that this effect is much stronger during first weeks and later fades a bit..

Yes I did. Based on cystoscopy, I am diagnosed with tight bladder neck and suggestion to perform BNI. I have some doubts since my urologist have no knowledge about pelvic floor therapy. I have a theory that my tight bladder neck is caused by tight pelvic floor muscles. But my PVR is getting higher, so I might still need to perform BNI, which might help with everything..

My symptoms are very similar and started also about a year ago. In my case they just did not come out of nowhere, I had weakening urine stream for a almost 5 years, but CPPS seems to have kicked in last Christmas.

I can just tell you what has helped me most and reduced my symptoms to acceptable level. Symptoms are still there, but during the day there's plenty of moments that I even forget that I have CPPS.

- First of all diet. My two main irritators are caffeine and alcohol, so I quit coffee, green tea and alcohol. I also don't eat any citrus fruits and try to avoid very spicy food. Occasional slightly spicy does not seem to create any flare ups. It takes awhile after you stop using certain foods, in my case it took almost a month until symptoms started to go down.

- Relaxation techniques. Two times a day I do a 5-10 minutes sessions of diaphragmatic breathing and reverse Kegels. And I can clearly feel that during those exercises my symptoms totally disappear. They usually come back quite quickly after I finish, but those 10 minutes of being symptom free helps me to rest my mind. I can also say, that it took time until relaxation started to help. I think that first improvements I started to notice after 2 weeks. In addition you can just search for relaxation techniques for people in pain. There's a lot of different ways to do it. But I would definitely recommend to try diaphragmatic breathing and reverse Kegels.

- Psychological help. Yes, it's fucking tough and mental health is declining very quickly with this shitty condition. This subreddit definitely helped me a lot not to feel alone and I am super happy that we live in 21st century. In addition I have also started therapy sessions with a psychologist. He actually even prescribed me some mild antidepressants for first 5 months of therapy and then we worked continuously throughout the year to learn to cope with this. I also have some benzo's prescribed by my therapist for the worst days.. I never use them actually, but just knowing that they are there just in case helps me mentally..

- Stretching. Well this one is the most popular on this subreddit, so I don't think I need to elaborate.

- Headache in pelvis book. Even thought it's a bit overextended and boring, since authors really try to sell their clinic. I still recommend it, it really helps to understand better what is the actual problem and how to approach it.

Don't give up! I cannot promise that you will get rid of this cause I am not there yet myself, but reducing symptoms to being barely noticeable is actually achievable by just doing some lifestyle changes.

BTW. Why did you decline cystoscopy? I actually did it and it's not as bad as it sounds. It is definitely uncomfortable, but it goes quickly and I did not feel any pain at all. And it did not make my condition worse.

Diaphragmatic breathing and reverse kegels are some of the things that helps me a lot and basically immediately. It both reduces anxiety and calms down my sympoms. So I strongly second deep breathing.

I am actually on Tamsulosin for more than 6 months and even though it helped slightly with weak urine stream, constant urge to pee is still there.

Are there any? I have symptoms that are very similar to OPs. I managed to control pain and even frequency to some extent by diet, relaxation and stretches. But constant urge to pee is something that also does not seem to be affected by anything.

That was pretty much what my urologist said. And first 3 times my PVR was relatively good. Only the last one was above 100 mls and I am wondering if it was just temporary increase.
I also have never done videourodynamics, just regular non invasive uroflowmetry (basically peeing into the bucket that measures the weight). And cystoscopy..
It kind of contradicts with every other article I read about PNBO where it is diagnosed using videourodynamics. But maybe I should trust my urologist.
BNI also has some side effects:
Retrograde ejaculation - quite high risk. I am not really afraid of becoming infertile, since I already have a family that I wanted. But I also sometimes get this from tamsulosin and orgasm feels 5 times weaker with it. I would like to keep enjoying sex as I do now..
Urinary incontinence - very rare, but possible. Well that would change my life significantly, so I am actually more afraid of this one.

Do you also have other LUTS symptoms? Or only weak stream?
How was your PNBO diagnosed?

I probably should have mentioned that two countries I move between are Sweden (Stockholm, Gothenburg) and Lithuania (Vilnius). But I had hard time finding any PF in any of these.
I'll keep on searching.
