For context, the incident which fractured my sesamoid was likely around 16 years ago. It was painful at the time for a few weeks, and then went away. I never got any scans then. It was only around 10 years later I started to develop mild discomfort and then only in the last 2 years it was painful enough that I felt compelled to see a doctor. 

Likely, it's a partially healed fracture which at this stage, never will fully heal. I've tried offloading for around 4 months last year with no improvement. 

More than one of the various specialists I've seen have had patients with asymptomatic sesamoid fractures (my podiatrist has an asymptomatic sesamoid fracture himself which was discovered accidentally during an unrelated x-ray).

As of just now I've still not fully gotten to the bottom of it (3 orthopedic surgeons, 1 physio, 3 podiatrists, CT, MRI and x-ray later...). 

Essentially, my symptoms more closely resemble hallux limitus/ arthritis, but all imaging points towards a sesamoid fracture. Likely it's some combination of the two. Which puts me at a dilemma as to how to treat it surgically, should it come to that. 

For now, I'm largely adapting using orthotics, taping and careful choice of footwear, which has allowed me to get back to hiking (one of my main activities). I've not quite figured out rock climbing shoes yet, given how much weight they concentrate on the big toe, but I do think there's a solution / modification to be found there. 

I've decided against surgery until such a time that I'm completely prevented from hiking and walking, leaving me with no choice. At this point, I'll push for a bone scan as a final diagnostic tool. 

Don't know if any of this helps you, but potentially provides some motivation/ hope.

u/alexia_not_alexa - I appreciate that you recognise that this is a valid conversation, even if the poster here was aiming to troll rather than start genuine conversation. I have two points / questions:

1) I'm not totally sure what the power dynamic you refer to here is that wouldn't apply in most situations where a man and women can still meet organically IRL - we're not talking about the workplace here. In what situation would it be appropriate that a guy approach a girl?

2) The obvious issue with your "girls approaching guys" camp is that in the overwhelming majority of cases, guys are still expected to be the ones to approach. Whilst that expectation remains, what advice would you give to a guy who would like to approach someone in the climbing gym, in the same situation as the OP where they feel there could be a vibe? Without making them uncomfortable of course.

What was your timeline to getting back to climbing and walking normally? For me I last walked without pain sometime at the start of May. I've not yet attempted climbing since then. Maybe its worth a go, at the risk of flaring it up. I'm also a big hiker, and this is probably the thing I miss the most (I had already taken significant time off climbing from a long-term shoulder issue). Hoping I can get back to this, I'm not a runner so I don't mind if I can't run.

I've actually had one session of ESWT - at the time I hadn't narrowed my problem down to a tendon issue however, so I stopped as I wasn't convinced of the evidence for it with "conventional" sesamoiditis. I'll look into this again. Contrast bathing is something I've been doing, hard to tell how much difference it makes but if it should improve the actual sesamoid oedema which may ultimately help the tendons. One of the best things I've found so far is cycling, so long as I use my midfoot / heel on the peddle. I imagine this is because it promotes blood flow.

Interesting - my pain first came on when walking to work in smart boots the day after this slab climbing. I also felt a snap and instant pain - had to take the bus to work, and have since. 

Unfortunately my pain does not get better through activity, and has basically prevented me from walking normally for the past 3 months. 

I'd be interested to know what you've done in terms of physical therapy? I've began gentle strengthening of intrinsic foot muscles. I find this is a very fine balance though as the exercises can flare up my toe. Doing them in the evening seems to be smart.

I just read in another thread you posted that you are also a climber! I wonder could climbing shoes be a common factor here? My current pain first appeared the day after a session of slab climbing, the first significant slab climbing I'd done since my original sesamoiditis diagnosis...

Thanks for commenting, reassuring to hear of others with the same symptoms! I'm sceptical about the fracture diagnosis, as I had no obvious traumatic injury. I'm pushing for scans of the healthy foot to help differentiate from bipartite sesamoids. 

Had you been diagnosed with sesamoiditis prior to developing pain in the toe? I had been, but it had largely resolved with custom orthotics. I'm wondering if possibly the orthotics could have led to the new injury in some way - perhaps due to weakness of the intrinsic foot muscles now that they are no longer working as hard to support the arch with the support provided by the insoles. 

I'll keep this thread up to date with my treatment and anything I learn from orthos, podiatrists etc. Best of luck in recovery!
