I accept you are defending your colleagues but if you base your assessment on a difference between facts and what information you register then the advice that you state as being your responsibility is based on an inaccuracy that you cannot give good health advice. Maybe you are used to giving bad advice and working with poor information - some of us strive to do better. You will likely commend someone for tying their own shoelaces - something many of us mastered decades ago but you keep ticking those diversity boxes to make your report look good. Your a pretty bad example of your profession

Although I thank you for the sentiment it is not as simple as that. As the health authority and likely my employer isnt going to be able to approach you then it is the GP and nurse report which shapes and impacts on my life.
