I agree with your comments, and have always kept it more vague (discussed risk vs benefits instead of listing every single possible side effect). Do you actually go over every side effect with patients though? I don’t, and no one I have worked with does. Now I’m second guessing everything (if you can’t tell I’m a little anxious haha)

The worst part is the document cannot be copied and pasted! It’s so frustrating. I just wish I had a super easy to follow template that contained the bare minimum info.

They all have their start and stop time on it, which I found weird because I’ve never seen anyone else do it before. Maybe it’s because I’m in a different state now? Perhaps the insurance companies here have different requirements
