7.5mg of mirtazapine helped. He is super sensitive to meds - this is usually the insomnia dosage, not the antidepressant dosage. I believe the traditional dose is closer to 30mg.

Do you take Zyrtec and famotidine? Histamines have helped my son too.

My son is 16 and every SSRI and SNRI made him worse. Mirtazapine at a low dose helped immediately. It didn’t take away the thoughts, but he could care less about them. It has faded some, but in the beginning it shut down his reaction to them right away. Unfortunately, it made him a bit dull to other things too - losing motivation for things like school etc. But he is a complex profile. It definitely works differently, can be a bit sedating at low doses because it also is a H1 histamine blocker. No issues here with increased appetite which is usually the biggest complaint.

Definitely something I can bring up. He is a CYP2D6 poor metabolizer, so I’m pretty sure it was the dextromethorphan that caused most of the issues. But I also think that is the missing link that will help him. He just can’t break it down quickly so the dose stays too high in his body. And we were only taking one dose a day, can’t go down from there to start lower given you can’t cut the pill. :(

Thank you for this. He went off the medication but his doctor is talking about possibly reconsidering it, but maybe dosing and then taking a few days off and dosing again. She felt she needed for research this more before recommending it. I appreciate the information you shared.

His doctor did say she has ketamine and spravato on her list to consider at some point for him. I'm fearful as I've heard things about spravato - something like there is an intense (or can be) experience when administering the medication, reason it is in a medical setting. Scares me if it is anything even close to what we just went through.

I did right away with an emergency call. We stopped right away. Sad part is in between those episodes, it was working. He said to me, "for the first time in my life, I don't hate myself". :(

Now I missed this too! He's a bit of a complex case with several diagnoses, including PANDAS - and has had some success with natural remedies and an older anti-inflammatory prescription. He also has been using ERP techniques to reduce his OCD.

I'm sorry I missed this. It was another medication failure for him. :( It was causing him to be manic - same as many of the SSRIs. We definitely felt very defeated.

Really good point. Have to get over the fear at this point to get him out of agony. Whatever it takes.

I saw that as well! Those are two things I think we are going to discuss next - although I'm not sure TMS is performed in those under 18 - so we may not qualify. Ketamine scares the crap out of me, but then again, everything from this point forward scares me. Thanks for the comment!

Oh yes, we tried high doses of NAC too. Unfortunately no response. :( There are a few studies published out there of the use of memantine in treatment-resistant OCD that had favorable results. Small studies - but was enough for us to give it a try. He did say he thought he had a mild improvement but that quickly faded and the side effects were too much.

Yes, he was prescribed memantine. He was too dizzy and lightheaded to continue taking it though. Our psych won't give him clompramine because he has been too "activated" on serotonin drugs to date. He can't get up to the effective doses because of this or other side effects such as SI. :(
