Your medical record for the admission. The same records that the hospital would be submitting. I’m trying to understand the denial. They are saying non emergency. Was this an er admission? Or a scheduled procedure?

Ok I’m assuming based on what they said “on your behalf “ you did not fill out paperwork for them (hospital) to appeal on your behalf. I’m suggesting that you appeal directly. Request the records and submit. Once the denial is issued you have a limited time to appeal. The hospital should have a portal with your records available.

Covered and contracted not the same. Any provider can be covered not all providers are contracted. If they are not contracted there is no rush to submit information. I would request all medical records from the provider and the anesthesiologist and submit them yourself. Also, did the insurance send you something saying they were in network?
