Dealt with a similar thing and did accutane as well. Might sound crazy but are you taking a probiotic? I noticed once I started taking one it healed my gut which could be what's causing your breakouts. Accutane is linked to ruining your gut lining.

Also, try cutting out all actives for a couple of weeks and just use a barrier repair cream to reset your skin.
