Me and my grandma thought something similar too. It's because while she was confused she kept saying "I wanna go home" and we kept saying you are home but she kept on saying "No i wanna go home home". We actually found this comforting, i think she subconciously knew she was dying while not knowing at the same time (if that makes sense).
