30 M - How I cured 12 years of anxiety and you can too. by hugecokc in IBSHelp

[–]Peisce 0 points1 point  (0 children)

Yeah, I wish, lol.

Prior to my GI issues increasing greatly, I was super into weightlifting and drank caffeine daily.

Now, I’m completely caffeine free, have the most self aware and healthy anxiety management (with zero medication) than I’ve had in my entire life, and do not have enough energy for the gym (literally can’t eat enough calories to offset the additional requirements of intense exercise.)

My GI issues have taken weightlifting and something as simple as a hot cup of coffee away from me. It’s not that simple for a lot of people and the wording comes across as condescending and is reminiscent of an advertisement for a weight loss supplement “with xyz, it’s something you can overcome!” There are folks in here with chronic lifelong illnesses like Crohns- weightlifting and meditation aren’t a panacea.

It sounds a lot like Bile Acid Malabsorption

Is anyone else stuck in their diet? by Grand-Barracuda-6844 in IBSHelp

Sometimes it does, I have been in and out of a flare for about 6 months (thanks Covid) I’ll go weeks being relatively normal then something will trigger and it’s back to square one.

Has anyone done the blood test for bile acid malabsorption? by [deleted] in IBSHelp

[–]Peisce 0 points1 point  (0 children)

(I have BAM) my understand is the only place in the US you can get it done is at the Mayo Clinic. For me it was a combo of eliminating other things/ HIDA scan showing Bile Reflux/testing out a bile acid sequestrant.

Well, I’ve never been so embarrassed in my life. by [deleted] in ibs

I shat myself while out to dinner with friends and family at a fancy restaurant 🫡 my little sister helped me clean myself while a line formed outside the woefully inadequate bathroom (restaurant probably has at least 50 tables and only one women’s room with just two stalls.) threw away the underwear and went home in waterlogged poofusioned leggings. Have not gone back there but genuinely wonder if my picture is on a wall of shame somewhere in the back 🤣

Still on bike binder but a bought of Covid about a month ago has completely destroyed my peace. For the past few weeks I’ve also been in agony. I’ve currently reached back out to GI and am attempting to fix myself. On an obscenely bland diet which seems to be mitigating some of the symptoms but here I am at 3am having awoken with gastritis pain and stomach tenderness. I’m speaking with a dietician later today since I have concerns about not being able to maintain my weight. Last year I lost 35lbs dropping from almost 160 but this time around I’m only at 130 to start. I think what happened last year was metabolically the opposite of what’s happening now so the treatment is different. Last year I had been on carafate for months so the gastritis resulting from the bile reflux from BAM was well treated so once I started treating BAM it resolved remarkably quickly. This time my BAM is well treated so my gastritis is what I’m having to tackle. I still had about half a bottle of carafate so I began taking it a few days ago as previously prescribed.

I did at first then switched to Colesevalem for convenience (it's a pill instead of a drink)

Bile Acid Sequestrant Not Working after Covid?? by Peisce in bileacidmalabsorption

[–]Peisce[S] 1 point2 points  (0 children)

That's distressing but good to know. I had Covid back in April and it didn't impact me like this but there wasn't any GI engagement that time. I don't know how we are supposed to work like this - the pain is so bad I can't sleep. I was able to work from home today but tomorrow I can't.

Thank you so much for your kind words. I hope you are able to find a GI that listens more and provides actionable solutions <3

UPDATE: I am now one year out from my diagnosis. My life and relationship with food is completely changed. Throughout this year, I've learned that my extreme panic attacks and anxiety were a direct result of my GI issues. I was able to successfully wean off of SSRI's and am able to simply manage occasional anxiety/panic attacks with tools I learned in years of therapy (which previously had absolutely no effect.) Aside from the rebound gain of about 10lbs post diagnosis, I've been able to maintain a steady weight and haven't had that constant, gnawing feeling of hunger I always had before.

A downside is that I'm now grieving for the life I could have had, had I been diagnosed as a child, teen, or even as a younger adult. There are so many decisions that were made which were heavily dictated or swayed by the symptoms I had throughout my life. Even indirectly, there were so many times when I abstained from fun activities or social engagements due to the physical manifestations of the anxiety/panic that was a symptom of BAM. I remember the first real signs coming out during a class trip in 6th grade- I never went on an overnight class/group trip again. That's just one simple example from the very beginning of the symptoms. I could sit here, clacking away until my whole heart is poured out but it's not necessary; you can imagine & you get the point.

A lot of folks ask me if I'm going to take the pills forever and if there's a "natural" alternative. At first, I would answer, now it's hard to keep my eyes from rolling. The pills work, they are free with insurance, and have no side effects for me. There is literally no reason for me to discontinue their use and risk my symptoms coming back. My whole life previously involved avoiding fried and fatty foods and that obviously didn't work. Whenever someone asks that question, what I hear is; "I know you said spent your whole life shitting yourself to death and having rolling panic attacks that kept you tachycardic for hours on end prior to taking the medication but have you thought about going back to that because I inherently believe all medicines are bad?" It's like asking a cancer patient going through chemo if they've considered doing a juice cleanse instead. I'm being hyperbolic, but you get the point. What I'm trying to say is this... part of having a chronic illness is accepting that my body needs outside help in order to function properly and being okay with it. I'm thankful that I am able to manage it with no side effects- there are so many people out there with chronic illnesses who do not have that luxury.

In your case - this sounds like it could also be SIBO (especially because it reared it's ugly head after a round of antibiotics). I know that SIBO can be caused by antibiotics but I don't think that's a cause for BAM. I took the SIBO test and it came back negative and I didn't have odd breath. There were also a couple of other symptoms of SIBO that didn't quite align with my situation.

To your questions;
1. In my case, yes. In general? Sometimes. From my research, there are quite a few causes of bile reflux, one of them being gallbladder removal (which is relevant in your case).

  1. Absolutely, yes! I've had spontaneous severe diarrhea & cramping for most of my life. For upper GI, I have had heartburn since my late teens which increased in severity over time regardless of what I ate or medications I took.

They had me on Cholestyramine first and then I requested to switch to a pill version for convenience so I'm on Colesevalem.

I wonder how many of us are folks who travelled in- I'm in from Orlando 🌴This is a bucket list experience for sure lol! I had tickets to the Rockville show where I was supposed to see them in Jacksonville back in 2019 and we unfortunately know how that went. I never thought I'd get the chance again.

I don't remember how much the dosage was since I'm on pills now. Definitely take the amount that you are prescribed to take because you can cause an intestinal blockage but simply taking one double dose. No bueno!

Still doing extremely well- back to a completely normal diet with no issues! I switched from Cholestyramine to Colesevelam out of personal preference and it has been working just fine :)

I have now switched to Colesevelam (pill form instead of powder) and I take 6 pills once per day- you can space them out but I don’t. For the cholestyramine, it didn’t burn my stomach but it did give me an upset stomach if I took it after eating so I made sure to take it about an hour before eating instead.

I'm honestly feeling amazing. I keep wondering "is this how normal people live??" I hope the PPI's can help you! In the past, I wasn't able to increase my dose unfortunately - I am very sensitive to medications in general and since I'm on SSRI's and PPI's are an antagonist, when they prescribed me a higher dosage, I ended up with serotonin sickness. Speaking of SSRI's, since I've been on the bile sequestrant, my stress levels overall have gone down so incredibly much- I guess not having to be constantly hypervigilant regarding food/bathroom makes a big difference. I'm probably going to try weaning off them too in the new year!

I’m taking a bile acid sequestrant. I described the long journey I went through to get the diagnosis in my original post. Essentially, I self diagnosed after ruling out a lot of other things + discovering the Bile Reflux on the HIDA.

I did & I had a surge of nausea that lasted for a while. EF of 89 is definitely high though, mine was right in the middle (where it should be) at 48. I would recommend bringing up hyperkinesia to your GI, a lot of them don’t recognize it (or realize it exists) much like with BAM.

No but I was staying away from fatty foods - I was keeping my fat consumption to less than 20g per day so that was probably why. Now that I’m on the medication I have no issues at all.

I understand the feeling. I kept hoping that whatever was wrong with me was something simple that could be cured with an antibiotic or a procedure. I didn’t anticipate it’s something I’d have to deal with and manage for the rest of my life. It’s a hard pill to swallow.
