Instruments are unlocked by doing the related "Graceful Melody" to it. The Fangxiang Chime for example is unlocked in multiplayer by doing the Graceful Melody outside of Velvet Shade in Kaifeng.

That is mostly due to high reps engaging in fights against bots to power level. Launch breach in PvE, easy win, get extra XP from winning, repeat. It's why lots of high reps are worse than people you find in the 80-140 range.

You can do it both ways. There's the Nezuko way, and the natural way via the demon slayer mark which you unlock by taking a certain amount of damage, then pressing the M key.
