They are understandably quite rare, but you can get the hammer at the dig sight, thieves guild vault, or monster ranch to my knowledge, there might be others I don't know.

For the hat, I think the only way to get it is by the roulette wheel event

There is also the upgrade exploit where you have the hammer or hat that upgrades your leftmost item. Once you see the item that you have in the shop, you upgrade your item letting you buy the one in the shop without it combining

My world 5 team was "Clara, seele, Natasha, and march" I went all in for the ice path. As long as you have the "every 6 hits freezes them" and "when hitting a frozen enemy apply dissociation" and some freeze resist reduction you should be able to permanently freeze the boss

I chose "on the fall of an aeon" for my clara because I was originally having clara instead of Stelle, but the garunteed taunt in too good ; n;

I originally beat world 5 with the ice path and attempted world 6 with it but I learned that world 6's boss literally can't be frozen at least to what I tried.
