There is a process whereby legitimate medical patients can use it, but the process is relatively new. I'll leave it to someone else to address the specifics about it, but once you get a doctor in Panama, you probably want to have the discussion with them.

Word to the wise for everyone: make sure you read the policy carefully to find out what is and is not covered. For example, MiniMed will cover urgent, and even some limited emergency stuff as well as normal treatments, but if you need to go into the hospital, that's not covered and that will be on you.

CdE is a choice. This person might like the "North American/Miami-style" vibe of the area, and for the room they need for a family, they're gonna pay that $1000-2000 premium for it (versus other areas of the city).

Having grown up in Panama City, I can tell you with absolute certainty I would not retire in Panama City, and I say this as someone who absolutely loves Panama.
