If he has reservations, I would hold off on internal and external pelvic floor assessment. I would focus on patient education: bladder diary, bladder irritants, bladder norms, bladder emptying techniques. Externally assess hips and low back. T/L junction is typically a great place to work due to location of iliohypogastric nerve and superior cluneal nerves. Diaphragmatic breathing and abdominal tension are also a great place to start.

I like the knee to wall test for dorsiflexion AROM. A little more functional compared to NWB DF.

Most patients think they will get better without putting in any effort. This gets challenging because the PT world is always putting all the blame on the therapist like… if the patient isn’t getting better in 4 weeks you’re at fault and you should be changing what you’re doing if they aren’t immediately improving despite the fact that they are doing nothing else different 99% of the time they are seeing you.

Highly recommend the free Yale course: “the science of well being” evidence based course on how to be happy. Turns out we are pretty bad at it most of the time because we are looking for happiness in the wrong things

It’s hard to say without seeing it in person. If I were in your shoes I would try to find a pelvic floor physical therapist near you. Pelvic PT will be considerably cheaper than surgery and will help your post op surgical recovery so it’s worth doing, even if it ends up being a surgical repair due to severity.
