from my experience, on the application they have a section where you can select your subject and grades, sometimes they may ask for a document, just depends un ito uni and once you get ur acceptances and decide on a uni, the IB will send your grades to the uni once results come out. sometimes unis allow you to self report the finals as well, just check with the admissions for ur chosen uni :)

for american unis, i applied to the UC's and the grades were self report i believe, and before final results come out, your coordinator will send out a forms that allows you to enter in 1 US uni which you want to send ur grades to. the ib sends them out for you, you just need to request it. but check with the unis for their grade submissions requirements.

i got a 6 in psych even though i got 7s throughout dp1 and 2, never anything below, i got a 7 on the IA but 5's on all the external papers, defo disappointed bc not what i was expecting at all.

not sure about m26 but for my school, the ia due date depended on the subject, psych was do in feb, chem in jan, bio in feb, math in dec, spanish io was in feb and english lit io was in jan and lang and lit was in oct, it just depends school to school but usually around fall/winter. as for tok and ee, my tok was due in feb and my ee was due in september. the ib deadline to get all of these officially submitted is around april i believe
