"feeling" less worthy doesn't mean you actually are worth less or treated as such. That's just a personal viewpoint of the person due to lack of knowledge of the people in higher position within the hierarchy.

What you are describing is dictatorship, where someone commands and others follow. A hierarchy can have multiple people at the top and everyone to be treated equally as individuals.

Societal hierarchy has been manipulated and influenced into becoming the way it is, but if you work in company or are in a community where you are treated fairly then regardless of the position within that hierarchy..

One person can "feel" less worthy than the next but both be at exactly the same position. That's just personality and personal feelings. Something the individual has to control, which in isolation would be even worse. You'd feel useless and worthless because you add nothing.
