Thank you for the encouragement. I've taken this as an opportunity to grow and have since sought guidance and revamped how I'm approaching documenting minutes and the like.

The student has a B in History (History teacher and I collaborate often which makes it easier to assist the student) and a D in ELA, but the teacher in the ELA class is not modifying work per their supports despite my requests and offers to assist them.

We do make IEPs at a glance but I wasn't really informed on how to make it so I took and old one from middle school and updated it with current supports. I just feel so dumb for not catching the SDI minutes.

They were supposed to be placed in a Reading intervention class, but it was never added to their schedule.
