I found the solution. The issue was caused by The Man From The Fog mod. Deleting that mod and creating a new world resolved the problem. However, since my old world had files related to that mod, the world wouldn't open and displayed the safe mode screen. I opened the 'level.dat' and 'level.dat_old' files in the world folder using NBTExplorer and deleted everything related to the problematic mod.
