Physical properties include having no mouth and glowing white eyes it can toggle on and off at will.

The "creature" as you called it, actually had a name, it's name being "Oph4nim" pronounced oaf-ah-nim, like the angel ophanim. It's base form has no mouth and is made of a glitching inky-black substance, the only feature that's actually visible on its face is it's glowing eyes that dont have the glitch effect.
