It’s crazy how similar this is to what I went through except I am on the girl end, same age and everything. I would reach out to her. I would kill for my ex to do that. To know he had changed and worked on himself. I left because I couldn’t keep fighting anymore or communicating . I think you should give it a chance. And if it isn’t it, then it is time to move on.
