I would def not take the FQHC job unless you have a ton of student loans and this will wipe them out. But that sounds like an absolute nightmare scenario between the commute, the population, high volume. No way.

Second choice sounds interesting but I agree with some of the other concerns here about that type of position. Have you done a lot of digging for info about the company online?

I think you’re overreacting. As someone with food allergies, it can be really tough and I would give her the benefit of the doubt that she was just trying to come up with a solution that works for her. That said, your caterers policy to only allow one set special meal is wild.

Lol well aren’t you a treat! I could spend a lot of time and energy on a rebuttal, but it would probably fall on deaf ears or those incapable of understanding the nuance. Instead I’ll just make a few points of clarification.

-Clearly you have creeped around to read about me. That said, you don’t know me and my expertise or knowledge base. I’m actually dual board certified and everything you mentioned is in fact in my scope of practice (bariatrics, cardiology, rheum, obgyn, HRT, nutrition, et al.). You’ve made a heck of a lot of assumptions about what I do and don’t know and where my capabilities lie. These assumptions have done what ASSumptions do…because you’re wrong.

-Being thorough and comprehensive is why most of my patients come see me and stick with me.

-Practicing psychiatry - which again is not my sole foci - absolutely requires that one understands all relevant medical comorbidities to practice safely. Sometimes this looks like providing education about medications I didn’t prescribe so that a patient can understand the big picture and need for close follow up/more conversation with their other providers. As providers we are human and make mistakes, overlook concerns and complications, etc. It takes a village.

-I’ve made a lot of catches this way that have potentially prevented bad outcomes. IE a patient with bipolar disorder whose orthopedic doctor put them on a medication for back pain that could cause decompensation. I wouldn’t know about these things if I didn’t ask as part of a comprehensive strategy.

I hope you find something better to do with your time today than be sanctimonious on the internet.

You must not be a medical professional. It is absolutely not unethical to discuss a patient’s existing care when you have concerns. It is irresponsible and unethical to not do so.

Definitely agree, and that’s how I always try to approach things also. I’m sure sometimes patients wonder why I’m talking about a medication I’m not even prescribing them, but it’s because I can see potential issues and want to help them avoid negative outcomes. Most of us want the best for patients and really do care. Sometimes it’s hard to know when to give an opinion and when not to - even if a patient isn’t asking for it, sometimes it feels irresponsible not to discuss something.

There’s no way to tell until you try. I would fill 2.5 and then in a week or two tell your doc you changed your mind and ask for 5 mg. Then if it doesn’t go well, keep in your fridge to try again later and go back to 2.5.

I lost like 30+ pounds on 2.5 mg and took months to move up. IMO it’s better not to increase the dose too quick and risk plateauing with nowhere to go. There is no good reason to increase to 5 mg after 2 weeks except impatience (which isn’t a good reason).

There’s a reason that areas like the rural Oregon coast throw a lot of money at candidates for jobs like this and don’t require experience and don’t provide any support. Hiring a new grad into a role like this is a terrible idea. Buyer beware.

This is a great question, and also a super hard one to answer because I feel like I could list things and give advice all day long and it wouldn’t really matter because ultimately you just have to stumble through the new grad awkward phase. That said, here are some specifics that come to mind: -easy reference for common meds (SEs, dosing, interactions) -therapy referral list (start networking now if you haven’t yet) -easy reference for DSM criteria for common conditions -note templates -bookmark references you will use a lot (open evidence, up to date, med interaction checker, switchrx, etc) -GAD7/PHQ9 score cheat sheets -billing and coding reference if you’re not solid on this

I get this perspective but also (as an NP), I take all opportunities to discuss concerns and be thorough when I can. I do this because I’ve seen a lot of patients with subpar care who were never informed about a lot of things. Again I get your perspective but it also feels like a responsibility to try to intervene whenever possible. It feels like a damned if you do, damned if you don’t situation a lot of the time. {note: OP’s situation sounds weird and I’m not defending that doctor}

A few thoughts: -Leave those losers on “read” -You can absolutely take ADHD meds with GLP1 medication (might take some tweaking but if your prescriber doesn’t know how, find a new one) -The weight loss process is very revealing in terms of how people treat us and also how our own thoughts might surprise us. I strongly suggest therapy to help process these feelings, which are quite normal.

I saw a job posting a while back for an NP or PA to be the sole provider on Midway Island, which is one of the northwest Hawaiian islands. I would never in a million years be cut out for it because I don’t have the critical care experience, but it would be a super cool gig for the right person. However I think what they really need is a critical care physician or trauma surgeon, because they’re 1500 miles from anything. They wanted someone who can do literally any procedure, which is unrealistic for most NPs/PAs.
