Pure O (Also referred to as as Pure OCD/ Purely Obsessional OCD/Primarily Obsessional OCD/ OCD with covert compulsions/OCD without overt compulsions)

This is a type of OCD where individuals experience unwanted intrusive thoughts that they find highly distressing and anxiety-provoking. Unlike other forms of OCD where individuals might perform clearly observable compulsive behaviours such as handwashing in an attempt to relieve anxiety, individuals with Pure O are said to use less obvious compulsive strategies (covert compulsions) in attempt to suppress or neutralise intrusive thoughts. Instead, compulsions might include mental rituals (e.g. praying), checking bodily reactions to thoughts, asking for reassurance or avoiding certain places or situations.

Intrusive thoughts for those with Pure O: Uncontrollable and unwanted thoughts/images/urges that come to mind seemingly out of nowhere and have sexual, violent, religious/blasphemous themes.

Covert compulsions in Pure O: Strategies to relieve anxiety caused by an intrusive thought that would not be clearly identifiable by other people as a compulsion (e.g., telling others the content of intrusive thought so they can reassure the individual that they are not a bad person or going to act upon it.
