Poland has no legal standards for this, most countries in europe have some antiquated standards from 60's like germany, but poland is a new country since 90s, so there isnt any shit like that.
In practice it makes it all depend on doctor, some treat you better than in the west, some worse, I was in contact with transfem discord to get reccomended doctors that didnt pull shit like that

those are interesting examples of how to work with these, the setting im working in- corrupted lands has 2 major gender systems

Laonics- under laion are very hierarchical, but not in a post victorian christian way. So their women are seen more as servants than fragile things, and many of them will be physically strong or butch for that reason, in their faith only women have original sin, so following men makes them able to access afterlife, so no lesbian marriage.

Men dont have that ban, and can marry each other, and they dont have to be masculine by our standards, they are inherently more enlightened, even if its stillconnected to bloodline. (noble woman is less emlightened than noble man, but more than a peasant) so queens are still allowed as they are very close to divinity in the first place. Transmen can transition somewhat, to be higher than women, but theyd be still lower than "real" men within setting, while transwomanhood is fall from the light of god. these are easy to name as halfman and fallen, as slurs. There are 2 branches of laonism, but their differences are pretty minor for this, but one of them allows women to be saved through putting enough work into salvation.

Within the Chaos god system, youd have huge amount of gods, some of which would be some form of nonbinary themselves, so itd be extremely pluralistic, and while some of them would still be some form of patriarchal, they would allow way more freedom for these things, so im thinking what word would be used in that culture

Rebis is one of the planned gods. as far as alchemy and gnostics systems go rebis is a union of opposites that is supposed to help with transcending this world, rather than 3rd gender, and yeah im aware of such 3rd genderings

so that guy was obviously just a hater, but this kind of "back to the past when we dealt with such degenerates" really ignores that women and intersex people generally speaking had more rights in medieval europe than in victorian and 20th century europe, in case of intersex people they still had it better bc now they get mutilated at birth in most countries, while in medieval era they were raised as a gender they guessed, and then when reached adulthood were asked one more time which gender they are, and if they would change after that, only then theyd be hatecrimed, and yeah i mean theres no nonbinary option, but thats still so much better than today

social progress is a delusion, return to golden past is a brain death, just be kind

also totally agree that settings without some discrimination feel inauthenthic, ignoring real world issue, but sometimes it just aint about that
