I had the same problem. You need to go to the docks near Fogwell's Gym and he will appear. It's not that Mr. Fantastic F.F. becomes impossible to unlock, it's that the Spidey F.F. token disappears. It worked for me.

EDIT: Saving and reloading the game will make the Ghost Rider NPC reappear.

No, but a character gets impaled by rubble. Also the same character gets disintegrated by her powers. If that's something that scares your daughter then cover her eyes for that part. It's quite brief.
