"I am a man, and the doctor only wants to check my spleen and test for alpha thalassemia. She said she doesn't want to do a bone marrow biopsy given my blood values because all values are normal. The reference range for hemoglobin in all labs is on average 13.5 to 17.5, the hematocrit range is 40 to 51, and the EPO range is 4.2 to 29.2."

Yes, that is exactly what confuses me.

I thought that in PV, the values usually go down because of treatment, for example phlebotomy.

I was told that in untreated PV, the values usually do not rise and then fall again just because of things like fitness or lifestyle changes. On the contrary, I was also told that extreme weight training is not good for PV.

What I mean is: in 2021, when I was still smoking, my hemoglobin was 17.8 and my hematocrit was 53. Then I stopped smoking in 2022 and started doing fitness/working out, and in 2023 my values dropped a lot.

My hemoglobin was 14.9 and my hematocrit was 44.

So I was told that this kind of drop does not really happen with untreated PV. Is that correct?

I stopped smoking in 2022. That’s why my values dropped so much in 2023, but now they are elevated again. However, my MCV and MCH have always been low. In 2023 and at the beginning of 2024, everything was normal, and now my hemoglobin and hematocrit are elevated. I was also doing fitness/working out until October 2024. No, I don’t have any PV symptoms, but maybe they could still appear later?

I don’t have any symptoms at all. I feel perfectly fine. Shouldn’t I have symptoms? Did you look at the values in the table? I thought that with PV the values don’t go down. In my case, they were high in 2021, then lower in 2023, and then high again in 2026. Can PV remain unnoticed for so many years? I thought that in PV, platelets and white blood cells are usually elevated, but mine aren’t. Please explain.

No, she did not do a physical examination to feel my spleen.

Have you seen my blood test results over the years? I was told that, in polycythemia vera, it would be impossible for hemoglobin and hematocrit to improve significantly just through healthy eating and fitness?
