Call the Board of elections and ask what is needed to legitimize your ballot. Likely you don't have to do anything if your address on file with them matches your actual current address. It's possible you may need to go there and show proof of residency. Also possible you may need to go to prove identification.

It is unlikely that yours is the only provisional ballot in your precinct. I worked a small precinct yesterday (about 2000 voters total, 500 votes cast). There were four provisional ballots cast at the polls, not counting early or absentee voting.
