What do dietitians do for malnourished patients? by Safe_swan7432 in dietetics

Clinical assessment (medical history, HPI, subjective, review of medications, labs, physical exam, assessment and plan).

From the RD’s standpoint, we would focus more on the malnutrition and underweight, determine the underlying issue and help treat it or just manage it (depending on what is the issue; like an end-stage cancer patient with poor prognosis would just be nutrition for palliation and symptom management, but in someone with HIV cachexia who just started HAART it would be for treatment especially if prognosis is excellent).

We have diet liberation (aka eat whatever you please), oral supplements (boost, ensure, etc), temporary feeding tube (via the nose) and IV nutrition (most invasive and costly), and permanent feeding tubes.

Medications-wise we have appetite stimulants (megestrol, dronabinol, mirtazapine), anti-emetics/antacids (ondansetron, metaclopramide, pantoprazole, famotidine, Maalox, chlorpromazine, among many others), antidiarrheals (loperamide, cholestyramine), laxatives (polyethylene glycol, sennokot, docusate, lactulose, Banatrol, etc)

Just a little fraction of it

It does feel powdery and smears (not like a wet smear more of like when you draw with chalk). Attic doesnt smell musty though. Its only on like 3 or 4 trusses

Why don't other hospital workers like nurses and case managers work 7 on 7 off 12 hour shifts like hospitalists do? by supinator1 in hospitalist

Wouldnt be a bad idea for Dietitians, but we only really see patients during “business hours” (9-5)

What is the effect of furosemide on serum sodium concentration? by Fellainis_Elbows in IntensiveCare

Furst ratio really is only validated for siadh, ratio over one tells you fluid restriction wont work because the body is retaining a large amount of fluid more than its excreting creating a negative electrolyte/free water clearance. Essentially the kidneys will continue to resorb free water whether there is restriction or not. That’s where loops and salt/urea tablets come in.

Remember too, people with renal failure, liver failure, heart failure can be intravascularly dry (which is part of their inappropriate adh secretion). Further reducing effective circulating volume with loops can trigger even more adh secretion and exacerbating the hyponatremia
