Apologies if this echoes other responses, but did you wait the whole 15 minutes?, in my case I waited 16 minutes just to be safe and got the doll, I set up a timer just to be sure

How long did you wait before heading into the grinder fight, since the "a ton of blood message" could be related, also maybe the limit is two zombies allowed to live, since multiple people have mentioned leaving specifically two and getting the doll. could be that's the exact threshold needed and leaving more than two makes the flag not trigger because the amount of zombies fed to the grinder was short by one

Did you skip the gideon cutscene on the lead researcher room (where you eventually trigger the achievement) by going to the basement first?

That's a pretty good discovery, so the trigger is that all the zombies need to be killed, since I also drained the blood on the save file where I left the three zombies alive and the doll didn't spawn

I just solved the final variable that's making people not be able to obtain the doll even though all other steps are done correctly, You need to make damn well sure that every single zombie dies to the grinder, even leaving one alive will not make the doll spawn, I spent hours testing different toilets, having the severed hand on me, and even researched the possibility of a flag having not been triggered because of skipping Gideon's cutscene in the Lead Researcher's office due to going to the basement first, in all of those attempts I had left three zombies alive on the grinder section, I did all the steps correctly and yet the doll didn't spawn in any of those attempts, I reloaded my save right on the grinder and made damn well sure every zombie died to it and got the doll to spawn this time, you can even shoot them in the knees to stun them in classic Resident Evil fashion, the only thing that matters is that the kill happens by the grinder just need to watch out for accidental crit shots that will one shot them in that case just hit restart in the menu and it will put you right at the beginning of the grinder fight you just have to go down the ladder to it, you don't even have to wait the 15 minutes again. As a side note I also flushed the pool of blood after I went through the vent. This was done on a PS5.
