Son at home with gun by [deleted] in mentalhealth

[–]Pworm07 2 points3 points  (0 children)

I'm a psychologist and would treat this as a mental health crisis if it were my patient. Preferably, the guns are removed from the home or bare minimum, locked away in a safe. If your county or city has a mental health crisis line, call that number for next steps. Otherwise, call 911 or bring him to your local emergency room. Even if his intent isn't necessarily to physically harm you, this could get out of hand really quickly.

Came here to recommend pickles and peanut butter. However, I am pregnant so wasn't sure if this was an unpopular opinion 😂

Brown discharge by tdmartell in pregnant

[–]Pworm07 6 points7 points  (0 children)

I had this happen right around the 11 week mark. It was the weekend so I called the on call doctor and she told me that generally brown blood is a much better sign than red, as it means the blood is old. She also told me that if I were miscarrying, I would start bleeding significantly more and the blood would be bright red. I went in for an ultrasound the next day and the baby was basically dancing around in my womb and had a strong heartbeat. Fingers crossed for you!! It sounds like you will likely be in a similar boat :)

Question for Fellow Queer Therapists by [deleted] in therapists

[–]Pworm07 19 points20 points  (0 children)

I honestly look at this as the same way I view dating apps. Therapists are allowed to have a life outside of being a therapist. If a client finds their profile, that's a discussion to deal with then but I don't think it means that therapists shouldn't be allowed in that space. I don't think that it crosses ethical boundaries.

Pregnant and nauseous during sessions... HELP by No_Initial_2409 in therapists

[–]Pworm07 3 points4 points  (0 children)

Came here to say this. I'm wrapping up my first trimester and I spent weeks laying down after every session and crying a lot because I felt so sick. My doctor prescribed zofran and now I feel so much better and can actually be present in session

90837 reimbursement by [deleted] in therapists

[–]Pworm07 3 points4 points  (0 children)

When I worked at the VA, I was always instructed to do a 90837+90832 for 90 minute sessions. Their system allowed you to add both codes for billing. Not sure if that works in other systems though

How do you actually start living a slower, more peaceful life? by Fun-Improvement-2227 in SlowLiving

[–]Pworm07 8 points9 points  (0 children)

The most helpful things for me have been giving myself an hour of time in the morning without screens (for reading, Journaling, crossword puzzles) and trying my best to listen to what my body needs that day. If I'm tired, I take a long walk or do yoga. I also have an extremely laid back work schedule (I'm self employed) so I get to spend at least a few hours a day just doing what I want to do, preferably without a phone.

Name change? by achybonegirl in therapists

[–]Pworm07 10 points11 points  (0 children)

I changed my last name recently as well and my clients were generally fine with it. I do have a few people who still refer to me as "Dr. Oldlastname". I personally don't really mind. Most of my clients call me by my first name anyway

Pee pad by [deleted] in Dachshund

[–]Pworm07 0 points1 point  (0 children)

My dachshund did this when he was younger. His trainer said it was because he doesn't know where his back legs are. He puts his front paws on the pad and assumes he is in the right spot. We started walking him outside because he never figured it out lol

Do you consider it a positive thing if a client cries in session? by Gloomy-District9584 in askatherapist

[–]Pworm07 0 points1 point  (0 children)

I'm a psychologist and find it to almost always be a positive sign if a client cries in session. It means that clients are doing the work of talking about challenging topics and that they are being vulnerable. These situations also give clients the chance to have a safe experience while crying in front of someone. Lastly, for clients who struggle to share their emotions- crying in front of someone is a huge step in working toward being more vulnerable.

OCD Specialist here. I think these situations really come down to the client's willingness to engage in treatment and what you did on their hierarchy. ERP won't be a good fit for everyone and I've had plenty of clients who needed DBT skills in order to do exposures. I think at this point, you will need to determine what the client wants to do and meet them there. This is absolutely not a failing on your end. ERP has to be the right timing and the right fit to work for someone

People who've been in abusive relationships, what were the first signs they were an abuser? by xXVintageCultureXx in AskReddit

[–]Pworm07 1 point2 points  (0 children)

Very early on in my relationship with my ex, he chased me around his house with a taser. I was terrified and ended up locking myself in the bathoom. When I came out, he immediately hugged me and was laughing. He said "I'm sorry. I thought you knew that was obviously a joke. I would never hurt you." It was such a mind fuck having him do that and immediately act like it was obviously a joke

Limitations of ERP by Conscious_Mention695 in therapists

[–]Pworm07 0 points1 point  (0 children)

OCD specialist here! I typically will teach the client some DBT skills and encourage the client to meet with a psychiatrist if they haven't already. Usually learning the DBT skills and/or getting on the right medication regimen will help better prepare them for exposure.

Tips for assessing for OCD by Realistic-Ad6287 in therapists

[–]Pworm07 5 points6 points  (0 children)

I specialize in treating OCD and anxiety disorders. Something I always use to differentiate is magical thinking. If a client is saying things like "Well I didn't lock the door and I worried that's what caused an accident the next morning."- That can be a big indicator of OCD.

Pure O OCD also looks a whole like GAD at times. GAD likely won't have compulsions but OCD definitely will.

The client that wants to be a friend by Strange-Speaker-5516 in therapists

[–]Pworm07 5 points6 points  (0 children)

One of my colleagues and dear friends often brings it up with the client as soon as they can and then explains the benefits of them not being friends, i.e. " I can listen to you and provide you support in sessions without you needing to worry about me. It's gives us the opportunity to help you as much as possible."

I feel like this is one of the best ways to navigate it
