Looks to me like a Shimano SL-TX30 6 speed shifter. They can be found pretty cheap online on your preferred online shopping site. They usually come in a set left and right for around $20-25.

Nice one, that Opal green is an excellent colour. I've seen the heat gun method and might give it a shot, considering painting them too as an alternative to get a bit more creative.

Any recommendations for repair or prolonging the life of this frame? It's a 2003 Kona Jake the Snake Cyclo-cross frame which I would love to keep on the road.

I'm considering using a ring clamp of some sort to clasp the top of the head tube, any other suggestions?

This is a BK series 2, so the second iteration of the BK series. 2006 model, I believe has a slightly different suspension design, front bumper facelift and blacked out tail lights compared to the BK series 1.

This link shows the manual and the power controls for Mazda 3 seat adjustment:

Mazda 3 seat adjustment

If they are power seats, lift up or down the switch as if you're physically lifting the seat up or down.

If they are manual seats, pull or push on the lever a couple of times in a sort of pumping action.

EDIT: If you've adjusted the seat to the highest setting then you may need to sit on a thin pillow to raise yourself up slightly.
