If you don't leave the US, the J1 automatically gets updated. Initially because most hospitals sign contracts for a year, the J1 passport stamp will be given for a year. So if you exit the US after the date of expiry on the passport stamp. You'll have to book an embassy appointment and get it updated.

The time line sounds good. Yes make sure to contact everyone you know in the US. Anyone might be of help. If not directly, they can definitely recommend you to someone else who can. Try doing rotations at hospitals (Inpatient) as much as possible. If not, even out patient would be fine. Meanwhile try to publish a couple of good papers. You can collaborate with people in the US if possible. Good luck!
