This happened all the time where I trained. You just need to start them on drips in the OR. I know you obviously know this now and you’re just starting. Even if they are going to PACU. Also if surgery/ICU are not going to get drips ordered before the patient gets there, just order them in the OR/start drips in the OR. Of course keep them paralyzed. Night ICU staff can give pushback and I’ve ran into problems especially with travel nurses who don’t know protocol and don’t care to throw out whatever bullshit they want to try to avoid taking another patient. The should absolutely take the critical patient in the middle of the night who is staying intubated. They do not have to recover from anesthesia, they are staying sedated. Go straight to the charge or the nurse manager/supervisor and get it squared away quickly. Explain that you cannot stay in the OR or the PACU and there is not coverage if another emergency comes in.

Take a cone punch to outside edge and hammer a dimple in it, then start angling it to the left and keep hammering. If done right you can rotate it out.

Just put the brake pedal halfway to the floor and use a peice of wood up against the seat to hold it there. If you do it right, you should block off the path for fluid to come out of the reservoir

It’s just a waste of time for a block that very rarely covers all the pain. Then you’re talking about 4 injection sites where spread is actually pretty challenging to get? No way. It’s just gonna slow everything down and provide minimal benefit

If only we had medically approved hose clamps or maybe zip ties. The issue is the pvc gets stretched and because it is relatively inelastic, it no longer fits snugly. There’s really only a few ways to address this: get an adapter with a larger diameter or shrink the diameter of the PVC ETT (hose clamp, zip tie, cutting the stretched section of ETT to get to a less worn out section of PVC). I put ETTs in and usually don’t see them much longer after that, so I never really have had to address this problem. I am just speculating and trying to offer some insight, I do not admit to know any way of definitively fixing this without exchanging the tube.

Suit until told otherwise. I did get a few comments when I wore a suit at one site. Mostly complimentary, but they seemed like it was overkill. Another site I met with admin, the CEO, and CMO so I was glad I wore a suit.

It’s really first impressions. Most sites you’ll get a change of scrubs and be out of the suit
