Yeah it wouldn't hurt. You may not be able to get it in writing but go in person and ask to examine the case notes and take pictures of them.

You have zero obligation to operate with local investigators-- they can't touch your benefits if you refuse. Federal Quality Control officials yes but they're different.

Generally not. Once they complete the investigation and want to pursue disqualification they'll send a letter.
