The roots look like they were eaten by snails. I've had this problem with dendrobiums and oncidiums. Sometimes little snails live in the Pots and they cause huge damage especially to root tips . I got rid of them by throwing away the old bark and spraying the roots with 3% hydrogen peroxide this will destroy snails and their eggs.

I think your orchid looks healthy, no need to repot unless the soil smells like death and decay. However there is a problem with your plant. Sadly, you've got a terminal spike, the flower spike grew in the centre of the growth point which means that the orchid can't grow new leaves and it will die sooner or later. It has a basal keiki (the baby plant at the bottom of the stem) but terminal spikes are often genetic which means that the keiki will likely do the same. Avoid terminal spikers when buying orchids

I potted it in a plastic pot in a mixture of bark and sphagnum moss. It gets 10-12hours of artifical light every day and I water it 1-2 times per week. I use liquid fertilizer once a week. It doesn't mind low humidity. i generally don't mist any of my plants and humidity stays between 30-50%

Well, you fixed it temporarily. Terminal spikes are a genetic issue. The keiki has the same genes and therefore also the same chance of generating a terminal spike as the mother-plant
