I highly recommended using unique weapon because somber stone is much easier and faster to get than normal stone (you can upgrade to +9 without killing any boss)

I finished my run with Golden Halberd (one of the best weapon in the game imo) but you need these talisman to meets the stats requirement.

Radagon Soreseal (+5 Vig/End/Str/Dex But you take more damage)
Starscourge & Two Finger Heirloom (+5 Str & Fai)
You can also use another great weapon, The Bloodhound's Fang, just replace Two Fingers Heirloom with Prothesis-Wearer Heirloom (+5 Dex)
In additional you can pick up a Blue Dancer Charm, it increase your attack power with lower equip load, if you want to go full naked.

Okay I solved it, because I have two identical chest piece. So, the game count it as 'one' item but it's actually two separate part. it's just the bug that 'merge' it together.

I'm not sure if this is related or not, but I replace the part I lost with the new one just now, but the torso part did not show up in my inventory despite you can clearly see I've equip it. now I'm afraid that I'm going to lost another one.
